Badge Application Form
In accordance with airport policies and Federal regulations, all airport workers (employees, tenants, contractors, etc.) must obtain and display an identification badge issued by the Houston Airport System when working on the airport site.
The same Form is used at all HAS facilities (Intercontinental /IAH; Hobby/HOU; and, Ellington/EFD). However, a separate Application is needed at each airport if you will be working at multiple HAS facilities.
You must complete and sign a Badge Application Form.
Depending on where you will be working in the the airport complex, prior to approval, you will be fingerprinted and must undergo a background and criminal history record check. Your employer will provide you with more information.
The Badge Application Form is a five (5) page document which consists of two major components/parts:

Completing The Application - Basic Steps
There are four basic steps to completing the application:
1.) First, you must complete the Applicant information. This is your personal information as the requesting badge holder. Depending on what purpose you are using the application for (new badge; renewal; lost badge replacement), you must complete different sections.

2.) After you complete the Form, you must sign it in ALL places designatedfor each section required.
3.) Take the Form to your Employer (the company you work for), who must complete and sign
Part 6 (31, 32, 33) or the Authorization portion of Badge Renewal Application Form.
4.) Your Employer will forward the Form to your Company's HAS Badging Sponsor, for final completion and signature of the Form.
5.) Once the Sponsor has signed the Form, it will be returned to you and you are ready to go to the Badging Office. See the Procedures section for information on what you need to bring and be ready for.
Remember, you will need two forms of identification, one of which MUST be a picture ID.
Using the Online Form
The Online Badging Form is an Adobe PDF document. Depending on the version of Adobe you have, you have several options for completing the Form.
Get Adobe Acrobat Tools
The FREE Adobe Reader version allows you to complete the form on the screen and print it but you cannot save your form. If you don't have Adobe Reader, click the link below for a free copy!

To save a copy of the completed form on your hard drive, and e-mail/forward the form to someone (e.g. your sponsor or employer), you must have Adobe Approval or Acrobat (full version) for these functions to work.
